  • Maintain a person-centered approach in all aspects of your job, providing consumers with choice and respect, ensuring consumer needs are addressed to the best of your ability, at all times.
  • Develop individual service plan and ensure completion of all necessary documentation. Reviews and evaluates effectiveness of plan with consumers as needed.
  • Maintain comprehensive progress notes for all consumers.
  • Maintain all service coordination records in participant’s file including all phone contacts, correspondence and reports.
  • Act as a liaison/coordinator with day programs, schools, vocational or work-related providers.
  • Completion of housing documents per Department of Health (DOH) requirements.
  • Assist consumers with arranging transportation, medical and clinic appointments and other needed services.
  • Maintain required face to face contacts and home visits with each consumer.
  • Ensure completion of all incident reporting per DOH regulation.
  • Assist with intakes and assessments for new consumers expressing the desire and need for services.
  • Complete billing checklist and records as required.
  • Bachelor’s Degree and (2) two years of experience providing case management/service coordination, and information, linkages and referrals regarding community-based services for individuals with disabilities.
  • Prior experience with the NYS DOH TBI Waiver program a real plus!
  • Competent oral and written communication skills.
  • Knowledge of community resources.
  • Effective use of time management skills.
  • Must possess a valid driver’s license, automobile insurance and have reliable transportation.

Our service coordinators:

  • Coordinate the individual service plan of each participant using a person-centered approach.
  • Ensure safety for each consumer to live independently in the community.
  • $45,000.00 – $52,000.00
